The Clinic by Cate Quinn

Meg was always overshadowed by her rock star sister Hailey. After she completed the takedown of a loan shark gang at the casino where she worked, she saw the TV news. Hailey had died at a posh rehab clinic near Seattle of a heroin overdose. She believes that Hailey was murdered-after all, heroin was not her sister's drug of choice, and they cremated her after a hasty autopsy was performed at the clinic. So Meg checks herself into the clinic, knowing that her knowledge of poker will aid her in figuring out whodunit by the "tell" of each suspect. Meg has addiction problems of her own, and when she meets her fellow patients, she has to pretend she belongs there while sniffing out the murderer.
 
The story is not only told by Meg but also by Cara, the manager, who gets very curious about the accounting records of the Swiss psychiatrist who runs the clinic. Nothing is as it should be here and secrets abound. This was a very original story which had no problem going over the top. Quin did a great job of ratcheting up the tension and delivering a super punch at the end. You can check in any time you want, but you can never leave? Find out.
